Description
Lateral profile of the hindfoot centers on the calcaneus, the tarsal bone forming the posterior weight-bearing prominence of the heel. Superior to the calcaneal tuberosity, the Achilles tendon inserts on the posterior calcaneal surface, while the plantar aspect of the tuber gives origin to the plantar aponeurosis and intrinsic foot musculature. Anteriorly, the calcaneus articulates with the cuboid at the calcaneocuboid joint, and superomedially it supports the talus across the posterior facet of the subtalar joint. A blue sock or protective wrap covers the skin envelope, but the osseous contour still reads clearly through the posterior and plantar heel silhouette. Heel anatomy is not subtle clinically. Point tenderness at the medial calcaneal tubercle points you toward plantar fasciitis, whereas pain and swelling just anterior to the Achilles insertion suggests retrocalcaneal bursitis or insertional Achilles tendinopathy, often accompanied by a posterosuperior calcaneal prominence (Haglund deformity). Trauma follows predictable lines too: axial loading can drive calcaneal fractures with loss of height and widening, and lateral wall blowout may impinge the peroneal tendons beneath the fibula.
