Trapezium anatomy dominates the palmar (anterior) aspect of the radial wrist, with the carpal tubercle projecting from the volar surface near the radial border of the bone. Medially, the trapezium articulates with the trapezoid, while its proximal facet meets the scaphoid; distally, the saddle-shaped surface for the base of the first metacarpal sits inferior to the tubercle and slightly ulnar. The tubercle rises as a palpable bony prominence, positioned superficial to the groove that guides the tendon of flexor carpi radialis as it passes toward its insertion on the bases of the second and third metacarpals. Small but specific. Clinically, the trapezium tubercle matters because it forms part of the radial attachment for the flexor retinaculum (transverse carpal ligament) and helps define the radial margin of the carpal tunnel at the level of the thenar eminence.
