Description
Locus coeruleus (cerulean nucleus) occupies the dorsolateral pontine tegmentum, forming a paired bluish-grey neuronal cluster immediately deep to the floor of the fourth ventricle. From a dorsal brainstem perspective, it sits superior to the facial colliculus region and adjacent to the sulcus limitans, lateral to the median sulcus that divides the rhomboid fossa. The nucleus lies medial to the cerebellar peduncles and close to the superior fovea, where the ventricular floor transitions toward the rostral pons and isthmus. Clinically, this noradrenergic nucleus anchors discussions of arousal, sleep-wake regulation, attention, and descending pain modulation, with relevance to opioid withdrawal physiology, anesthetic depth, and neurodegenerative change in disorders such as Alzheimer disease and Parkinson disease. Its proximity to the ventricular surface also matters in dorsal pontine and fourth ventricle surgery, where a transventricular corridor risks injury to the pontine tegmentum, producing autonomic instability, disturbed vigilance, or abnormal pupillary responses.
