Emerging in the dorsal pontine tegmentum, the cerulean nucleus (locus coeruleus) appears as a paired, blue-gray cell group situated lateral to the midline on the rostral floor of the fourth ventricle. The sequence tracks its position relative to the rhomboid fossa, the superior cerebellar peduncle laterally, and the periaqueductal gray as the view advances rostrally toward the caudal midbrain. Subtle rotations clarify the anterior to posterior depth of the nucleus within the tegmentum rather than at the ependymal surface. Orientation is explicit. Clinically, the locus coeruleus is the dominant noradrenergic nucleus of the brain, with diffuse projections that shape arousal, attention, stress responses, and descending pain modulation. Neurodegeneration and tau pathology in this region are discussed in Alzheimer disease and Lewy body disorders, while altered noradrenergic tone is relevant to delirium and to perioperative sedation targets that act through brainstem arousal networks.
