Description
Anterior cervical alignment is presented from C1 through C7, with the atlas (C1) positioned superiorly beneath the occipital condyles and the axis (C2) immediately inferior, its odontoid process projecting superiorly behind the anterior arch of C1. Inferior to C2, the vertebral bodies of C3 to C7 stack in the midline, separated by intervertebral discs, with uncovertebral margins (uncinate processes) rising along the superolateral borders of each body. Laterally, the transverse processes extend from each level, framing the intervertebral foramina at the junction of adjacent pedicles and uncovertebral joints. Two vertebrae are color-highlighted in blue to isolate the level of interest against the remaining cervical bone. An anterior view like this helps clarify what clinicians target and protect during an anterior cervical discectomy and fusion (ACDF): the disc space, vertebral endplates, and the anterior vertebral body contour used for plate positioning and fluoroscopic level confirmation. The uncinate processes and uncovertebral joints are key landmarks for foraminal stenosis, where osteophytes often narrow the neural foramen and contribute to C5 to C7 radiculopathy. Small details matter. A misidentified level is a preventable complication.
