Posterior to the glenohumeral joint, the circumflex scapular vein is traced as it curves around the lateral border of the scapula and passes through the triangular space to join the subscapular venous system. Scapular landmarks such as the spine and inferior angle sit medial to the vessel’s course, while the proximal humerus lies lateral and slightly anterior where the vessel approaches the axillary region. Portions of the ribs and posterior thoracic wall frame the field, with adjacent arterial companions rendered in red to clarify the venous anastomosis around the scapula. Course through the triangular space matters because it fixes the vessel’s relationship to the borders formed by teres minor superiorly, teres major inferiorly, and the long head of triceps laterally, a small corridor that becomes clinically relevant in posterior shoulder approaches and when interpreting dilated collateral pathways. When axillary vein outflow is compromised or altered after trauma or surgery, the scapular venous network can enlarge, and the circumflex scapular vein becomes a recognizable route linking posterior thoracic and axillary drainage.
