Description

Inferior to the body of the mandible, the submandibular triangle is bounded anteriorly by the anterior belly of the digastric, posteriorly by the posterior belly of the digastric and stylohyoid, and inferiorly by the intermediate tendon at the hyoid level. Within this space, the submandibular gland occupies a superficial position beneath the investing layer of deep cervical fascia, while the mylohyoid forms the muscular floor and separates it from the more superiorly placed sublingual gland in the oral cavity. Medial to the gland, the hyoglossus and styloglossus provide a deep boundary where the lingual nerve and submandibular (Wharton) duct course toward the sublingual caruncle. Close quarters. For head and neck teaching, this triangle is the cleanest way to orient relationships that matter in the clinic: the facial artery typically grooves the posterior aspect of the submandibular gland before winding onto the face at the inferior border of the mandible, and the marginal mandibular branch of the facial nerve is at risk during gland excision. Sialolithiasis commonly obstructs the submandibular duct at the hilum or along its anterior course, and the pain pattern makes more sense when you can track the duct’s path relative to the mylohyoid and the lingual nerve loop. Surgeons also use this anatomy when planning a transcervical approach to the submandibular gland and when avoiding hypoglossal nerve injury deep to the posterior belly of digastric.
