Description
Centered on the thorax of a male child, the rib cage frames the mediastinum while semi-transparent soft tissues reveal overlapping organ systems. Within the costal margins, the heart sits slightly left of midline with the great vessels coursing superiorly toward the thoracic inlet, and paired lungs occupy the right and left pleural cavities lateral to the mediastinum. Intercostal muscles span between adjacent ribs, and segmental neurovascular bundles track along the inferior border of each rib, closely applied to the internal thoracic wall. Pediatric thoracic anatomy carries its own teaching points. A relatively large thymus commonly occupies the anterior superior mediastinum in boys and can alter the silhouette of mediastinal contours on chest radiographs, a frequent source of confusion for trainees. The tight spatial relationship between internal thoracic vessels, intercostal nerves, and the rib margins also explains why tube thoracostomy is directed just superior to the rib below to reduce the risk of injuring the intercostal artery and nerve. Small structures, small margins.
