Description
Running inferior to the clavicle at the junction of the deltoid and pectoralis major, the deltopectoral groove (deltopectoral sulcus) forms a distinct anterior surface landmark on the shoulder and upper chest. Laterally, the deltoid muscle belly defines the groove’s border as it tapers toward the deltoid tuberosity, while the clavicular head of pectoralis major forms the medial border as it courses toward the humerus. Deep to this interval lies the clavipectoral fascia enclosing pectoralis minor and subclavius, with the deltopectoral fossa representing the superficial depression near the infraclavicular region. Skin and subcutaneous contours in a neutral standing pose make the groove readable as an external guide. Clinically, this sulcus is the map for the deltopectoral approach to the proximal humerus and shoulder, favored for open reduction and internal fixation of proximal humeral fractures and for certain shoulder arthroplasty exposures. Surgeons develop the plane between deltoid and pectoralis major to protect the cephalic vein, which typically occupies the groove before piercing the clavipectoral fascia to drain into the axillary vein. Misidentifying the interval can shift dissection too lateral (risking deltoid splitting and bleeding) or too medial (obscuring the vein and limiting access). A clean landmark.
