Dorsal anatomy of the adult male second toe (index toe) is presented with the distal, middle, and proximal phalanges aligned along the longitudinal axis of the digit. The distal phalanx sits most anterior in the dorsoplantar plane, while the proximal phalanx lies proximally at the metatarsophalangeal joint line, with the middle phalanx interposed at the proximal interphalangeal joint. Along the dorsum, the extensor expansion and terminal slip of extensor digitorum longus would be expected to course centrally toward the base of the distal phalanx, flanked by the thin dorsal soft tissues and nail complex distally. This dorsal perspective matters when teaching how toe joints and extensor mechanisms relate to common traumatic patterns, because fractures of the phalanges and dorsal avulsion injuries often track the line of pull of the extensor tendon at the distal interphalangeal joint.
