Description
Across the dorsal midfoot, the navicular sits proximally, just distal to the talar head, and spans medially toward the tuberosity while tapering laterally to meet the cuboid. Distal to it, the medial, intermediate, and lateral cuneiform bones form a stepped row, with the medial cuneiform most prominent and the lateral cuneiform aligning toward the cuboid on the lateral column. The bases of the first through fifth metatarsals begin immediately distal to this tarsometatarsal line, widening proximally and creating the low, segmented contour of the dorsum of the metatarsus. Small surface mounds matter here. Dorsal midfoot anatomy is where clinicians localize Lisfranc joint injury, because subtle widening between the base of the first and second metatarsals or malalignment at the cuneiform-metatarsal articulations can signal instability even when swelling obscures the plantar findings.
