Centered on the posterior scalp, the occipital bellies of the occipitofrontalis (epicranius) sit over the occipital bone and converge superiorly into the epicranial aponeurosis (galea aponeurotica), which continues anteriorly beyond the field of view. Laterally, the muscular fibers taper toward the superior auricular region, where small slips can blend with the posterior auricular muscles and the mastoid fascia. Superficial scalp layers cover the epicranium, while deep to the aponeurosis the loose areolar tissue forms a plane of mobility over the pericranium. Midline landmarks dominate. Posterior orientation matters because the occipital belly anchors scalp tension and explains why lacerations that cross the galea gape and bleed profusely, while more superficial cuts can look dramatic yet remain mechanically stable. Elevation of the scalp in neurosurgical and craniofacial approaches exploits the subgaleal plane, and this view clarifies how a subgaleal hematoma can spread widely over the calvaria but is limited anteriorly by the orbital attachments and posteriorly by the nuchal lines. Occipital neuralgia and targeted greater occipital nerve blocks are planned relative to this same posterior scalp anatomy, even when the nerve itself lies deep to the illustrated epicranial layers.
