Description
Dorsal and plantar contours of the adult male foot (pes) are presented with emphasis on surface anatomy and the supporting arches. The hallux lies most medial, with digits II to V extending progressively laterally, and the forefoot transitions proximally into the midfoot at the tarsometatarsal region. The calcaneal prominence forms the posterior heel, while the medial border tracks along the navicular and first metatarsal line that corresponds to the medial longitudinal arch. Lateral to it, the fifth metatarsal tuberosity and cuboid region define the lateral column and the lateral longitudinal arch, with the transverse arch spanning across the metatarsal heads. Pedal arches are not abstract concepts; they map directly to load transfer during stance and gait, and their failure patterns explain common clinical presentations. Flattening of the medial longitudinal arch (adult acquired flatfoot, often from posterior tibial tendon dysfunction) shifts weight medially, alters hindfoot alignment, and drives pain around the navicular and medial ankle. At the other extreme, a cavus foot concentrates pressure under the metatarsal heads and lateral border, predisposing to metatarsalgia, stress injury, and recurrent ankle sprain. Surface landmarks also guide examination for plantar fasciitis at the medial calcaneal tubercle and for trauma at the base of the fifth metatarsal.
