Description
Dorsally and plantarly, the male third toe (digitus pedis III) is presented as the central ray of the forefoot, aligned between the second and fourth toes and continuing proximally into the forefoot at the level of the metatarsophalangeal region. Beneath the skin envelope, the phalangeal skeleton is implied, with the proximal, middle, and distal phalanges arranged in series and joined by the proximal and distal interphalangeal joints. The nail plate caps the distal phalanx on the dorsal surface, while the plantar pad thickens over the distal tuft and along the flexion creases. Medial and lateral nail folds, the pulp, and the toe’s longitudinal axis help establish orientation relative to the foot’s midline. Surface anatomy of the third toe matters because subtle malalignment here often reflects forefoot mechanics rather than an isolated digital problem, and it is easy to miss when attention is fixed on the hallux. Hammertoe or claw toe deformity typically presents as hyperextension at the metatarsophalangeal joint with flexion at the interphalangeal joints, producing dorsal callosities over the proximal interphalangeal joint and plantar pain under the metatarsal head. The third toe is also a common site for interdigital (Morton) neuroma symptoms in the adjacent web spaces, and for trauma patterns such as distal phalanx crush injuries with subungual hematoma.
