Description
Presented in a dorsal (superior) view, the right foot and distal leg are rendered with high color contrast to isolate surface anatomy. Across the dorsum of the midfoot and forefoot, the extensor hallucis longus tendon tracks to the hallux while the extensor digitorum longus tendons diverge distally toward the lateral four toes, lying superficial to the tarsometatarsal joints. Medially, the contour of the medial malleolus and the course of the great saphenous vein region are implied, while laterally the lateral malleolus region frames the ankle, with the dorsum tapering distally to clearly separated digits and nail plates. Clinically, this viewpoint supports instruction and documentation of palpable landmarks on the top of the foot, including the dorsalis pedis artery pulse just lateral to the extensor hallucis longus tendon as it crosses the talonavicular region. Small errors in spatial relationships matter here. Pain and swelling over the tarsometatarsal complex after a twist or axial load raise concern for a Lisfranc injury, and dorsal swelling can obscure the extensor tendons and superficial fibular (peroneal) nerve branches that supply cutaneous sensation over much of the dorsum. The dorsal approach also underpins operative exposure for metatarsal fixation and midfoot arthrodesis, where tendon planes and neurovascular corridors guide safe dissection. Ideal applications include lower-limb surface anatomy lectures, podiatry and orthopaedic teaching files, and patient-facing materials that explain where clinicians palpate pulses, assess edema, or map dorsal cutaneous sensation.
