Description
Dominating the posterior hip and buttock region, the gluteus maximus appears as a broad, thick sheet of fibers spanning from the posterior ilium, dorsal sacrum, and coccyx to the proximal femur and iliotibial tract. Its superior fibers run laterally toward the iliotibial band, while the deeper, inferior fibers course more distally and laterally to insert on the gluteal tuberosity of the femur, lying superficial to the short external rotators and posterior capsule of the hip. Inferiorly, the muscle overlaps the proximal posterior thigh, where it relates to the hamstring origin at the ischial tuberosity. Bulk matters here. Posterior orientation gives the clearest read on how gluteus maximus contributes to hip extension and external rotation during gait, rising from a flexed position, and stair climbing, and why weakness produces a compensatory posterior trunk lean (gluteus maximus lurch). Clinically, its position as the main superficial landmark for safe intramuscular injection makes surface anatomy non-negotiable, with the upper outer quadrant used to reduce risk to the sciatic nerve and the inferior gluteal vessels.
