Description
Beneath the posterior skin of the male pelvis, the gluteus maximus forms a broad, diamond-shaped sheet spanning from the posterior ilium near the iliac crest and posterior superior iliac spine down toward the gluteal fold and proximal femur. Medially, its fibers approach the sacrum and coccyx, while laterally they thicken toward the iliotibial tract and the posterior border of the tensor fasciae latae fascial sleeve. Inferior to the muscle belly, the proximal hamstrings, biceps femoris laterally with semitendinosus and semimembranosus more medially, emerge along the posterior thigh and frame the ischial region deep to the gluteal mass. The proximal femur sits deep and anterior to these muscle planes. Clinically, a posterior view with the gluteus maximus still superficial gives a clean map for safe intramuscular injection placement in the superolateral quadrant, away from the sciatic nerve and inferior gluteal vessels that course deep to the muscle near the greater sciatic foramen. Hip extension and external rotation mechanics also read well here, because the fiber direction and distal pull toward the iliotibial tract versus the gluteal tuberosity explains why gait deficits appear after inferior gluteal nerve injury. A good landmark view. Surgeons will also recognize the surface anatomy that guides posterior and posterolateral approaches to the hip, where splitting gluteus maximus in line with its fibers limits denervation and helps protect deep neurovascular structures.
