Description

Centered within the true pelvis, the uterus (womb, metra) sits posterior to the urinary bladder and anterior to the rectum, with the fundus directed superiorly and the cervix projecting inferiorly toward the vaginal canal. Laterally, the uterine tubes extend from the uterine cornua toward the ovaries, which lie on the pelvic sidewall near the ovarian fossa and the internal iliac vessels. Red and blue vascular channels track along the broad ligament region, consistent with the uterine artery and accompanying veins approaching the uterus near the level of the cervix, while green neural elements suggest the hypogastric plexus contributions running medially toward the pelvic viscera. Posteriorly, the sigmoid colon continues into the rectum in the midline. Topography matters here. The uterine artery crosses superior to the ureter near the lateral cervix (the classic water under the bridge relationship), a surgical hazard during hysterectomy and uterine artery ligation for postpartum hemorrhage.
