Description

Centered in the male abdomen, the large intestine is traced from the cecum in the right lower quadrant, up the ascending colon to the hepatic flexure, across the transverse colon, and down the descending and sigmoid segments toward the rectum in the midline pelvis. A red, irregular neoplastic mass expands the colonic wall along the mid transverse colon, positioned inferior to the stomach and anterior to the pancreas, where the gastrocolic ligament normally drapes over the bowel. The lesion narrows the lumen and thickens the bowel circumference, contrasting with the smoother proximal and distal colon. Transverse colon tumors often present later than left sided lesions because the lumen is wider and stool is more liquid, so anemia, weight loss, or vague abdominal pain may precede obstructive symptoms. Location matters for surgical planning: a mid transverse mass pushes you toward an extended right hemicolectomy or transverse colectomy with central ligation along the middle colic vessels, and it raises specific nodal basins along the superior mesenteric distribution rather than the inferior mesenteric chain typical of sigmoid disease. Staging hinges on depth of invasion through muscularis propria into pericolonic fat and on regional lymph node involvement, the same features assessed at colonoscopy with biopsy, CT for metastatic spread, and pathology after resection. Margins matter.
