Anterior Region of the Ankle, Gross Anatomy

Description

Anterior to the tarsus, the distal tibia and fibula frame the ankle mortise around the trochlea of the talus, with the medial malleolus positioned medial and slightly anterior to the lateral malleolus. Across the front of the joint, the tendons of tibialis anterior, extensor hallucis longus, and extensor digitorum longus course distally toward the dorsum of the foot, held close to the joint line by the extensor retinacula. The dorsalis pedis artery and deep fibular (peroneal) nerve run in the anterior interval between extensor hallucis longus (medial) and extensor digitorum longus (lateral). Skin surface contours over the anterior ankle crease and the bony malleolar landmarks remain visible in this standing, anterior perspective. Clear landmarks. Clinically, this anterior corridor is where clinicians palpate the dorsalis pedis pulse and where the anterior ankle arthroscopy portals (anteromedial and anterolateral) are placed relative to the tibialis anterior tendon and the superficial fibular nerve branches. Tightness or thickening of the inferior extensor retinaculum and adjacent fascia can contribute to anterior ankle impingement and, more proximally within the anterior tarsal tunnel, compression of the deep fibular nerve. The fibulotalar relationship also matters when teaching ankle instability patterns, since lateral ligament injury alters talar tilt and anterior translation at the mortise.
