Description
Anterior thoracic anatomy centers on the paired mammary glands overlying the pectoral region, with lobules of glandular tissue draining via lactiferous ducts toward the nipple and areola in the midclavicular line. Fibrous suspensory (Cooper) ligaments tether the gland to the dermis, while the retromammary space separates breast parenchyma from the pectoral fascia covering pectoralis major and the ribs and intercostal muscles deep to it. Superficial and deep vascular channels accompany the gland, with perforators from the internal thoracic vessels medially and branches of the lateral thoracic and thoracoacromial systems laterally, and sensory supply tracking with anterior and lateral cutaneous branches of the intercostal nerves (classically T4 at the nipple). Green lymphatic channels converge toward axillary nodes and parasternal (internal mammary) nodes along the sternum. Clear landmarks. Breast lymphatic drainage patterns are the practical reason this view matters: upper outer quadrant lesions often spread first to pectoral (anterior) axillary nodes, while medial tumors may preferentially reach parasternal nodes, a pathway that can change staging and radiotherapy fields. The close relationship to pectoralis major fascia also frames the surgical planes for mastectomy, implant-based reconstruction, and the expected routes of local invasion, and it explains why chest wall fixation on exam signals deeper involvement. Venous and lymphatic congestion patterns correlate with peau d’orange and inflammatory breast carcinoma, and the ductal tree provides the anatomic substrate for ductal carcinoma in situ and pathologic nipple discharge.
