Description
Viewed from the inferior aspect, the diaphragmatic surface of the myocardium dominates, with the posterior interventricular sulcus running toward the apex and separating the inferior walls of the right and left ventricles. The right coronary artery courses in the coronary (atrioventricular) sulcus toward the crux cordis, where the posterior descending (posterior interventricular) artery typically descends in the midline groove, accompanied by the middle cardiac vein. More leftward, the circumflex branch of the left coronary artery tracks in the left atrioventricular groove beneath the left atrium, while the coronary sinus and tributary cardiac veins form a broad venous channel across the posterior heart. Superiorly, the aortic root and proximal great vessels sit at the base, partly framed by pericardial reflections. An inferior coronary view matters when you need to teach, or plan around, the coronary dominance pattern and the territories implicated in inferior wall infarction. Occlusion of the right coronary artery or, in left-dominant hearts, the circumflex can compromise the AV node region near the crux, a common mechanism behind bradyarrhythmias seen with inferior STEMI. The relationship of epicardial vessels to the myocardium and pericardium also helps clarify why pericardial inflammation can refer pain and why postoperative adhesions can make re-entry hazardous.
