Deltopectoral Triangle, Gross Anatomy

Description

Centered on the anterior shoulder girdle, the deltopectoral triangle (trigonum deltoideopectorale) is defined laterally by the anterior border of the deltoid and medially by the superior fibers of pectoralis major, with its base along the clavicle. The deltopectoral groove runs inferiorly between these muscle bellies and leads into the interval deep to the skin where the cephalic vein typically ascends toward the deltopectoral triangle before piercing the clavipectoral fascia. Deep to the superficial contour, the fascia invests pectoralis minor and continues as the costocoracoid membrane toward the coracoid process, a palpable landmark just inferior to the lateral third of the clavicle. Clear boundaries. Reliable surface anatomy. Surgeons and proceduralists care about this region because it is one of the few places where a major superficial vein tracks predictably toward the axillary vein without crossing a joint. The cephalic vein cutdown in the deltopectoral triangle remains a standard option for cardiac device lead placement and central venous access when percutaneous subclavian puncture risks pneumothorax. Orthopedic exposure through the deltopectoral interval (between deltoid and pectoralis major) is also the workhorse approach for open reduction of proximal humeral fractures and shoulder arthroplasty, and it demands respect for the deltoid’s innervation (axillary nerve more distal) and the anterior circumflex humeral vessels near the surgical neck.
