Description
Visible at the distal foot are digits I to V (hallux through fifth toe), each with its phalanges arranged from proximal to distal and aligned along the metatarsophalangeal joints. The hallux presents a proximal and distal phalanx, while the lesser toes show proximal, middle, and distal phalanges with interphalangeal articulations stepping distally toward the nail plates. Dorsal skin, nail beds, and the distal pulp are rendered with a diffuse blue discoloration that is most evident over the tips of the toes and around the periungual tissues. Darker baseline pigmentation is preserved in the surrounding pedal skin. Methemoglobinemia is a classic reason to emphasize peripheral color change in the digits, because the toes sit at the far end of the arterial tree where reduced oxygen delivery and abnormal hemoglobin chemistry become visually obvious. Blue toe and nail-bed discoloration from methemoglobin can be mistaken for simple acrocyanosis, Raynaud phenomenon, frostbite, or acute limb ischemia, but the distribution here favors a systemic dyshemoglobinemia rather than a segmental vascular occlusion. Teaching point: patients often have low pulse oximetry readings that do not correct with supplemental oxygen, and a chocolate-brown arterial blood appearance with an oxygen saturation gap on co-oximetry, often after exposure to oxidant drugs (for example benzocaine, dapsone, or nitrates). This matters in triage. Use this artwork in gross anatomy labs when introducing digital phalanges and joint terminology, then pivot directly into clinical correlation during emergency medicine, anesthesia, or toxicology sessions on methemoglobinemia and methylene blue therapy (and its limitations in G6PD deficiency). It also fits case-based teaching for dermatology and vascular surgery texts where you need accurate toe anatomy plus realistic skin-color change on a Black female model.
