Description
Centered between the costal margins and the umbilicus, the anterior epigastric region is framed superiorly by the xiphoid process and inferiorly by the upper linea alba as it approaches the umbilical ring. Paired rectus abdominis bellies sit on either side of the midline, with the linea semilunaris marking their lateral border and the external oblique forming the anterolateral abdominal wall. Deep to this surface map, the left lobe of the liver occupies the right and midline epigastrium, while the stomach (fundus and body) projects predominantly to the left, with the pylorus and proximal duodenum crossing near the transpyloric plane. Clear midline orientation matters. Clinically, the epigastrium is where patients localize pain from peptic ulcer disease, gastritis, and pancreatitis, and it is also where a pulsatile mass may be appreciated in a thin patient with an abdominal aortic aneurysm. Surface landmarks in this region guide safe placement of laparoscopic ports and upper midline or subcostal incisions, helping you anticipate where the falciform ligament, left lobe of liver, and anterior gastric wall will lie relative to the linea alba. Epigastric hernias through the linea alba typically present between the xiphoid and umbilicus, so accurate depiction of the midline aponeurotic anatomy supports diagnosis and operative planning.
