Posterior elbow anatomy is centered on the olecranon process of the ulna, which lies superficially at the proximal forearm and forms the posterior apex of the cubital region (cubitus). Proximal to the olecranon, the distal humerus contributes the medial and lateral epicondyles, with the olecranon fossa situated superior to the ulnar trochlear notch when the joint is extended. Medially, the groove behind the medial epicondyle marks the course of the ulnar nerve, while laterally the radial head sits distal and slightly anterior relative to the capitulum, inferred from the lateral contour of the joint. A posterior perspective matters because many high-yield clinical findings live on this side of the joint. Ulnar nerve irritation in the cubital tunnel is provoked by elbow flexion and traction at the medial epicondyle, and this view makes the relationship between the nerve’s sulcus and the bony landmarks unambiguous. Olecranon bursitis, whether traumatic or inflammatory, presents as posterior swelling directly over the olecranon, and a clear depiction of that subcutaneous bursa region helps distinguish it from deeper joint effusions.
