Description

Centered in the thorax, the heart sits in the middle mediastinum between the right and left lungs, with the right atrium and right ventricle positioned more anteriorly and the left ventricle forming the left inferolateral cardiac border. Superiorly, the trachea descends in the midline and bifurcates into the main bronchi at the carina, each bronchus entering its lung at the hilum along with the pulmonary arteries and veins. Pulmonary parenchyma fills the pleural cavities laterally, while faintly rendered upper abdominal viscera, including the liver, stomach, and intestinal loops, establish the inferior relationship of diaphragm to thoracic contents. Orientation is clear. Coupling the cardiac pump to the respiratory surfaces matters most when teaching cardiopulmonary physiology and its failure modes in older adults: elevated left atrial pressure transmits backward into pulmonary veins, raising capillary hydrostatic pressure and producing interstitial then alveolar edema. That path is the anatomic basis for orthopnea and bibasilar crackles in left-sided heart failure, and it also explains why cardiomegaly and pulmonary venous congestion travel together on chest radiography. The proximity of bronchi and pulmonary vessels at the hilum also frames common bronchial pathology (aspiration, obstructing carcinoma) and the downstream effect on ventilation-perfusion matching.
