Seen from the lateral chest wall with the upper limb slightly abducted, the surface contours trace the rib cage from the axillary region to the costal margin, with the intercostal spaces defining the thoracic breadth. The anterior axillary fold corresponds to pectoralis major as it wraps toward the humerus, while the posterior axillary fold reflects latissimus dorsi and teres major over the posterolateral thorax. Along the midaxillary line, the digitations of serratus anterior typically create subtle oblique ridging over ribs 1 to 8, blending inferiorly with the external oblique at the flank; posteriorly, scapular borders and the underlying ribs relate to trapezius and the upper fibers of latissimus dorsi. Skin tone and sex-specific habitus are represented, with the breast region covered by an undergarment while the lateral thoracic wall remains readable. Lateral thoracic anatomy is where many clinicians place their hands and instruments, and small errors in surface landmarking have consequences. Chest tube thoracostomy is classically performed in the safe triangle near the 4th to 5th intercostal space at the anterior to midaxillary line, where you aim just superior to the rib to avoid the intercostal neurovascular bundle in the costal groove. This is also the standard zone for auscultation of the right middle lobe and lingula from the side, and for recognizing flail segments or rib fracture tenderness along the lateral arcs.
