Description
Beneath a semi-transparent dermal layer, the longissimus cervicis appears as narrow, vertical muscle bundles running along the posterior cervical spine, positioned medial to the more lateral longissimus capitis and superficial to the deeper transversospinalis group. Fibers course superiorly from upper thoracic transverse processes toward the posterior tubercles of the mid-cervical transverse processes, paralleling the vertebral column in the posterior neck and upper back. The spinous processes form the midline reference, with the erector spinae mass flanking them bilaterally in a three-quarter posterior view. Segmental organization is easy to read. Longissimus cervicis matters when you need to explain cervical extension and ipsilateral lateral flexion as functions of the intermediate erector spinae column, and when you want a clear landmark between superficial extensor bulk and the deeper facet-spanning stabilizers. In posterior cervical approaches, subperiosteal dissection and retraction of the paraspinal musculature influence postoperative axial neck pain, and this view helps clarify what tissue planes are being elevated off the laminae and articular processes. Palpation and trigger point referral in paraspinals can be taught more convincingly when the muscle’s longitudinal fiber direction and proximity to the cervical transverse processes are explicit. Simple, but frequently misunderstood. Use this illustration in gross anatomy and kinesiology teaching to contrast longissimus cervicis with semispinalis cervicis, multifidus, and splenius, and to orient learners to posterior cervical landmarks before discussing imaging or surgical exposure.
