Description
Posterior neck anatomy is rendered with the cervical spinous processes centered in the midline from the occiput through C7, continuing into the upper thoracic vertebrae. Lateral to this column, the middle scalene is positioned deep to more superficial posterior cervical musculature, with its inferior attachments running toward the first rib, just lateral to the transverse processes. The base of the skull (occipital bone) sits superiorly, while the scapulae and upper ribs frame the lower field laterally and inferiorly. Deep planes are suggested beneath a semi-transparent soft tissue layer. Middle scalene anatomy matters because it defines part of the interscalene interval, where the brachial plexus roots and trunks course between the anterior and middle scalene on their way to the axilla. A small change in muscle bulk or tone can be clinically meaningful. Hypertrophy, spasm, or fibrous bands at the scalene triangle are implicated in neurogenic thoracic outlet syndrome, and the first rib insertion becomes a key landmark when correlating symptoms with posture, cervical rib variants, or provocation maneuvers. For proceduralists, recognizing the scalene relationship to the cervical transverse processes and first rib helps orient safe trajectories for ultrasound guided interscalene brachial plexus block and for surgical decompression planning.
