Description

Along the posteromedial ankle, the calcaneal tuberosity and Achilles (calcaneal) tendon form the dominant posterior contour, with the tendon descending centrally to its insertion and the pre-Achilles fat pad lying deep to it. Anterior to the Achilles and just inferior to the medial malleolus, the flexor retinaculum spans the tarsal tunnel, where the posterior tibial tendon runs most anteromedially, followed by flexor digitorum longus, the posterior tibial artery and veins with the tibial nerve, and flexor hallucis longus positioned most posteriorly. Distal to the malleolus, the medial aspect of the talus and adjacent tarsal region sit deep to these tendons as they curve toward the plantar foot. Blue tape is applied proximally on the distal leg. Posterior ankle anatomy is often taught from an anterior or lateral standpoint, but the medial view clarifies tendon layering and neurovascular relationships that drive real pathology and operative planning. Compression beneath the flexor retinaculum produces tarsal tunnel syndrome, and this orientation makes it easier to trace where the tibial nerve divides into the medial and lateral plantar nerves relative to the posterior tibial artery. The same corridor matters when evaluating posterior tibial tendon dysfunction, where pain and swelling track posterior and inferior to the medial malleolus, and when distinguishing it from Achilles tendinopathy or insertional bursitis at the calcaneus. Landmarks are unambiguous.
