Description
Positioned on the posterior thoracic wall, the triangle of auscultation is marked as a shallow, triangular depression just medial to the inferior angle and medial border of the scapula on each side. Its medial boundary follows the lateral margin of trapezius, the inferior boundary aligns with the superior border of latissimus dorsi, and the lateral boundary tracks the medial scapular border as the scapula rests over the ribs. Deep to the skin, the floor corresponds to the rhomboid major overlying the intercostal muscles, with the underlying costal surfaces and pleura lying immediately anterior to that muscular layer. Bilateral regions are indicated for left and right comparison. Clinically, this is the classic window for posterior lung auscultation because the overlying muscle layer thins when the scapula is protracted and the arms are brought forward, moving the scapula laterally and opening the space between trapezius and latissimus dorsi. Breath sounds from the lower lobes, including posterior basal segments, can be assessed here with less soft tissue attenuation than adjacent scapular or paraspinal areas. Small changes matter. Fine inspiratory crackles from early pneumonia or dependent atelectasis, and diminished breath sounds in pleural effusion, are often easiest to appreciate when the stethoscope is placed precisely within this triangle rather than over the scapula or thick latissimus.
