Description
Shown in a male lower limb with the pelvis and proximal femur included, the vastus medialis is presented on the anteromedial thigh as the medial component of quadriceps femoris. Its muscle belly lies medial to vastus intermedius and deep to the course of sartorius, then narrows distally into the quadriceps tendon adjacent to the medial femoral condyle. Proximally, fibers arise along the medial lip of the linea aspera and spiral anteriorly; distally, the muscle approaches the patellar region, with the tibia and fibula positioned inferior to the knee joint line. Emphasis on vastus medialis matters because its distal oblique fibers (often taught as the VMO) provide a medial vector on the patella and help counter lateral patellar tracking during knee extension. This relationship becomes clinically concrete in patellofemoral pain syndrome and recurrent lateral patellar subluxation, where quadriceps imbalance, increased Q angle, or postoperative inhibition after ACL reconstruction can leave the medial retinacular and vastus medialis complex under-recruited.
