Description
Oriented from a posterior thoracic viewpoint, the diaphragmatic surface of the heart rests immediately superior to the central tendon of the diaphragm, with the ventricular base abutting the muscular dome. Posterior cardiac contours logically include the left atrium and pulmonary venous entries, while the great vessels are seen superiorly, including the ascending aorta transitioning toward the arch and the pulmonary trunk with proximal pulmonary arteries. Inferiorly, the diaphragm slopes upward bilaterally, its peripheral muscular fibers converging toward the centrally placed fibrous center. Close relationships. Tight spacing. Clinically, this posterior relationship explains why diaphragmatic (inferior) myocardial infarction can present with epigastric discomfort, nausea, or referred pain patterns that overlap with diaphragmatic irritation. The course of the phrenic nerves along the fibrous pericardium, and their proximity to the cardiophrenic angle, matters in cardiac surgery and pericardiocentesis planning, because pericardial manipulation or inflammation can trigger ipsilateral diaphragmatic paresis. For teaching, the view clarifies how pericardium, myocardium, and diaphragmatic musculature share a functional interface during respiration and venous return.
