Description
Rendered in lateral profile, the hindfoot and heel of a Black adult female are emphasized, with the calcaneus forming the posterior and inferior buttress of the foot. The lateral malleolus of the fibula lies superior and slightly anterior to the calcaneal tuberosity, while the talus sits superior to the calcaneus at the subtalar joint. Posteriorly, the Achilles tendon tracks down the posterior aspect of the ankle to insert on the calcaneal tuberosity; inferiorly, the plantar soft tissues thicken under the calcaneal fat pad. A blue band encircles the midfoot, crossing lateral to the cuboid region. Lateral heel anatomy matters when sorting out hindfoot pain that is not classic medial plantar fasciitis. Peroneal tendinopathy or subluxation occurs along the posterolateral ankle, often related to retinacular injury behind the lateral malleolus, and symptoms can be mislocalized by patients as calcaneal pain. The lateral calcaneus is also the corridor for the extensile lateral approach to calcaneal fracture fixation, where respect for the sural nerve and the lateral calcaneal artery influences incision placement and flap viability.
