Description
Calcaneal tubercle anatomy dominates this medial view of the hindfoot, with the calcaneal tuberosity forming the posterior, plantar prominence of the heel. The medial process of the tubercle sits inferior to the posterior surface of the calcaneus where the calcaneal (Achilles) tendon inserts more superiorly, while the plantar surface transitions anteriorly toward the calcaneal sulcus and the anterior articular region. Just superior and anterior to the tubercle, the sustentaculum tali projects medially as a shelf for the talus, and the groove for the flexor hallucis longus tendon is suggested along its inferior aspect. Rough cortical texture at the plantar heel is a key landmark. Heel pain often localizes to this exact region. Repetitive traction at the medial calcaneal tubercle from the plantar aponeurosis and intrinsic foot muscles (including flexor digitorum brevis) correlates with plantar fasciitis and plantar calcaneal enthesophytes, while tenderness more posterior and superior shifts suspicion toward insertional Achilles tendinopathy or retrocalcaneal bursitis. This fixed medial perspective clarifies the relationship between the tubercle and sustentaculum tali, useful when teaching why tarsal tunnel symptoms cluster posteromedial to the ankle and why medial calcaneal nerve branches can be irritated by local swelling.
