Description
Along the ulnar side of the palm, the hypothenar eminence forms a soft tissue mound distal to the pisiform and proximal to the base of the fifth metacarpal. A colored overlay marks this region overlying abductor digiti minimi, flexor digiti minimi brevis, and opponens digiti minimi, which sit superficial to the palmar aponeurosis and the hypothenar compartment fascia. Medially, the contour blends toward the ulnar border of the manus and the little finger rays, while laterally it transitions toward the central palm and, farther radial, the thenar eminence. Skin creases and the thick glabrous palmar skin frame the surface anatomy. Clear and local. Clinical teaching often hinges on this exact surface landmark because the ulnar neurovascular bundle courses into the hand at Guyon’s canal between the pisiform and the hook of the hamate, immediately deep to the proximal hypothenar soft tissues. Compression here produces ulnar tunnel syndrome with sensory symptoms in the ulnar one and a half digits and weakness of intrinsic muscles, including the hypothenar group, a pattern that differs from cubital tunnel involvement at the elbow. Repetitive trauma to the hypothenar eminence can also injure the ulnar artery (hypothenar hammer syndrome), making this region relevant to vascular assessment in cyclists, machinists, and workers using impact tools. Hand anatomy modules, surface anatomy labs, and OSCE stations can use this illustration to anchor palpation of the pisiform and to orient learners to the ulnar palmar corridor before discussing nerve testing and intrinsic muscle function.
