Description

Posterior to the male large intestine, the inferior mesenteric vein ascends in the left retroperitoneum, collecting tributaries from the descending colon and sigmoid colon before coursing superiorly toward the splenic vein at the level of the pancreas. Along its path it parallels the inferior mesenteric artery, with the left colic vein and sigmoid veins converging medially as they approach the root of the mesentery. Haustra and epiploic appendages along the colon provide surface landmarks, while the rectum lies inferiorly in the midline and the transverse colon crosses superiorly. Posterior orientation matters here because it clarifies how the inferior mesenteric vein relates to the posterior abdominal wall and the planes surgeons enter during left-sided colonic mobilization. During a medial-to-lateral approach for sigmoid colectomy or low anterior resection, the IMV is often identified near the ligament of Treitz and divided high to improve exposure and lymphadenectomy, but its proximity to the inferior mesenteric artery and the pancreas makes the anatomy unforgiving. This is a common source of bleeding and anatomic disorientation. The venous pathway also explains portosystemic consequences when portal venous pressure rises, with backpressure transmitted to hindgut tributaries.
