Description
Seen from the posterior scalp and upper neck of an adult male, the occipitalis overlies the nuchal region while the upper trapezius descends inferolaterally toward the acromial area, framing the deeper suboccipital compartment. Deep to the superficial fascia, the inferior obliquus capitis (obliquus capitis inferior) runs from the spinous process of the axis (C2) superolaterally to the transverse process of the atlas (C1), forming the inferior border of the suboccipital triangle. Medial to it lie the rectus capitis posterior major and the interspinous midline landmarks, while superomedially the rectus capitis posterior minor approximates the posterior arch of C1 and the inferior nuchal line. Orientation here matters because the inferior obliquus capitis acts as a practical landmark for the C1–C2 interval and for structures that become symptomatic in posterior neck pain syndromes. The suboccipital nerve (dorsal ramus of C1) traverses the triangle in close relationship to the vertebral artery as it courses in the groove on the posterior arch of C1, and irritation or entrapment in this region is discussed in cervicogenic headache and occipital neuralgia workups. Palpation and injection techniques often reference the line between the mastoid region and C2, but the clearest anatomic logic comes from understanding how the C2 spinous process anchors the inferior oblique and how that muscle relates to the greater occipital nerve emerging inferiorly from C2.
