Description
Inferior surface anatomy of the cerebellar hemisphere centers on the lobulus semilunaris inferior (inferior semilunar lobule), a broad crescent of cerebellar cortex positioned posterior to the horizontal fissure and lateral to the inferior vermis. From this inferior perspective, the folia of the posterior lobe sweep obliquely across the hemispheric undersurface, with the semilunar lobule occupying the posterolateral field as it approaches the cerebellar margin. Medially, the vallecula and vermian contour provide orientation, while the tonsillar region sits more anteromedial on the undersurface, separating the semilunar territory from the midline. Posterior lobe topography matters when localizing cerebellar signs and planning posterior fossa corridors, because hemispheric lesions in this region more often produce limb ataxia, dysmetria, and impaired rapid alternating movements than primary gait imbalance from vermian involvement. On the inferior surface, the relationship of the inferior semilunar lobule to the horizontal fissure helps you distinguish posterior lobe cortex from adjacent lobules during gross anatomy review and when correlating surgical anatomy with MRI in the axial and coronal planes.
