Description
Viewed from the lateral aspect beneath intact skin, the muscular layers of the male thoracic wall and anterolateral abdominal wall are arranged to emphasize the internal oblique (musculus obliquus internus abdominis) deep to the external oblique. Its fibers course superomedially from the iliac crest and thoracolumbar fascia toward the inferior borders of the ribs and the linea alba, contrasting with the inferomedial grain of the superficial layer. Superiorly, pectoralis major occupies the anterior chest, while serratus anterior forms digitations along the lateral ribs; intercostal muscles fill the interspaces, and latissimus dorsi sweeps posteroinferiorly over the flank. For teaching layered abdominal wall anatomy, the internal oblique is the hinge point between surface landmarks and deeper surgical planes. Its aponeurosis contributes to the rectus sheath, and its inferior fibers help form the conjoint tendon, a key structure when discussing direct inguinal hernia repair and reinforcement of the posterior wall of the inguinal canal. Nerve relationships matter here too, since the iliohypogastric and ilioinguinal nerves travel between internal oblique and transversus abdominis, where they are vulnerable during lower abdominal incisions. Clean fiber direction tells the story.
