Description

Presented in an anterior, weight-bearing stance, the knee region centers on the patella (kneecap) seated within the quadriceps tendon and aligned over the distal femur. Inferior to the patella, the patellar ligament tracks to the tibial tuberosity on the proximal tibia, while the medial and lateral patellar retinacula blend with the joint capsule along the parapatellar margins. The femoral condyles create the rounded contours superior to the joint line, and laterally the fibular head sits distal and lateral to the tibial plateau as an external landmark. Anterior surface anatomy matters because so much of the knee examination is done by palpation before imaging ever enters the conversation. Patellofemoral pain syndrome, chondromalacia patellae, and lateral patellar instability all hinge on patellar tracking relative to the trochlear groove, with the lateral retinaculum and quadriceps vector often driving the patella laterally in extension and early flexion.
