Description
Lateral funiculus of the medulla oblongata occupies the white matter column between the anterolateral sulcus (ventrolateral root exit zone of the hypoglossal nerve) and the posterolateral sulcus (postolivary sulcus, root exit zone of glossopharyngeal, vagus, and cranial accessory nerves). From this lateral viewpoint, the funiculus sits posterior to the inferior olivary prominence and lateral to the medullary pyramid, with its dorsal aspect approaching the region that continues into the inferior cerebellar peduncle. Major longitudinal fibers course through this sector, including the anterolateral system (spinothalamic tract), dorsal spinocerebellar and cuneocerebellar pathways, and descending hypothalamospinal sympathetic fibers, organized as compact bundles rather than isolated nerves. Borders matter. Clinically, the lateral medullary territory is a frequent point of reference for Wallenberg syndrome from posterior inferior cerebellar artery or vertebral artery infarction, where involvement of the anterolateral system and descending sympathetics produces contralateral pain and temperature loss with ipsilateral Horner syndrome. The fixed lateral perspective also supports teaching of surface to deep correlations, linking the sulcal landmarks and olivary region to the tracts that sit just deep to the pial surface.
