Description
Positioned at the lateral canthus (commissura palpebrarum lateralis), the superior and inferior eyelid margins converge to form the lateral palpebral commissure at the outer corner of the palpebral fissure. From a lateral head view in anatomical position, the commissure sits anterior to the lateral orbital rim and the temporal region, with the upper lid curving slightly superior and posterior as it approaches the canthal angle while the lower lid meets it inferiorly. Just deep to this surface landmark lie the lateral palpebral ligament attaching to the zygomatic bone and fibers of orbicularis oculi, while the lacrimal drainage puncta remain medial and out of the field. Clinical work around the lateral canthus demands exact surface orientation because small shifts in canthal position change eyelid apposition and corneal exposure. Lateral canthotomy and cantholysis for orbital compartment syndrome are performed at this site, and the direction of the cut relative to the lateral canthal tendon and lateral orbital rim determines how effectively intraorbital pressure is decompressed. It is also the reference point when planning canthopexy or canthoplasty in lower lid laxity, ectropion, and post-traumatic lid malposition. A small landmark. Big consequences.
