Rising from a superior view of the diencephalon, the lateral posterior nucleus of the thalamus is positioned lateral to the lateral dorsal nucleus and anterior to the pulvinar, forming part of the posterior thalamic group along the dorsolateral thalamic surface. Medially, its border approaches the region of the third ventricle, while laterally it grades toward the internal capsule and adjacent thalamic radiations. The sequence typically advances through subtle rotations and depth cues to clarify how the lateral posterior nucleus nests between pulvinar posteriorly and the lateral dorsal nucleus more rostrally on the thalamic roofline. Landmarks that commonly contextualize this territory include the interthalamic adhesion (when present), the stria medullaris thalami along the dorsomedial margin, and the posterior pole of the thalamus. Clinical relevance tracks with connections rather than gross shape. The lateral posterior nucleus participates in higher-order visual-spatial attention and multimodal association networks via parietal and occipital cortices, so vascular lesions in the posterior thalamus, often in the posterior cerebral artery perforator territory, can produce contralateral sensory disturbance with neglect-like or visuospatial deficits that look cortical at first pass.
