Description
Positioned on the lateral aspect of the male hip, the gluteus minimus occupies the deepest gluteal layer, arising from the external surface of the ilium inferior to the gluteus medius and coursing inferolaterally toward the anterior facet of the greater trochanter. Superiorly it sits just beneath the iliac crest region, while its tendon passes anterior to the femoral neck and lies deep to the larger gluteus medius and gluteus maximus, which form the more superficial buttock contour. The greater trochanter and proximal femur provide the lateral bony landmarks, with the acetabular region of the pelvis positioned medially and slightly anterior relative to the trochanter. Deep and small, but easy to miss. Gluteus minimus matters because it is the anterior partner of gluteus medius in hip abduction and pelvic stabilization during single leg stance, and its anterior fibers contribute to hip internal rotation, a concept often tested when teaching gait mechanics. Pain over the greater trochanter is frequently grouped under greater trochanteric pain syndrome, and pathology can involve gluteus minimus tendinopathy or partial tears at the trochanteric insertion, sometimes mistaken for isolated bursitis on clinical exam. Surgeons also encounter this plane during lateral and anterolateral approaches to the hip, where protecting the gluteal tendons helps reduce postoperative abductor weakness and Trendelenburg gait.
