Description
Seen from the left lateral aspect, the left lung lies superior to the dome of the diaphragm, with the inferior border of the lung tapering down toward the sharp angle of the costodiaphragmatic recess. Along the medial surface, the root of the lung can be inferred where the left main bronchus and pulmonary vessels enter at the hilum, while distal airway branching terminates in alveoli within the spongy parenchyma. Inferiorly, the muscular diaphragm forms a convex boundary between the thoracic cavity and upper abdomen, its fibers curving upward under the lung and deepening the pleural recess posteroinferiorly. That recess matters clinically. Pleural fluid collects dependently, so the costodiaphragmatic angle blunts on upright chest radiography and ultrasound windows often target this space to confirm a pleural effusion, guide thoracentesis, or detect a small hemothorax after trauma; the lateral view clarifies why the needle trajectory is planned just superior to the rib to avoid the intercostal neurovascular bundle while still accessing the pleural space above the diaphragm. Phrenic nerve dysfunction also localizes here, because paralysis of the hemidiaphragm alters the resting contour and excursion, producing basal atelectasis and dyspnea that are often worse supine. A tight anatomic relationship.
