Presented in anatomical position, the anterior lower leg is centered on the tibia, with the sharp anterior border (tibial crest) running inferiorly toward the medial malleolus while the fibula sits lateral and slightly posterior, its head and shaft outlining the lateral column of the leg. Overlying the tibia, the tibialis anterior muscle belly occupies the anterolateral surface and tapers distally into tendons that pass across the anterior ankle, accompanied more laterally by extensor digitorum longus and extensor hallucis longus. The lateral compartment forms the contour of the calf’s outer edge with fibularis (peroneus) longus and brevis, while the medial surface remains relatively subcutaneous along the tibial shaft.
