Description

Posteriorly oriented lungs fill the thorax on either side of the vertebral column, with the right lung broader and the left lung narrower to accommodate the cardiac impression and mediastinum. Inferior lobes dominate this view, their posterior basal regions descending toward the diaphragmatic surface, while the oblique fissures sweep inferoanteriorly from the upper thorax toward the costodiaphragmatic recess. Along the midline, the trachea transitions into the main bronchi at the level of the carina, deep to the upper mediastinum and anterior to the thoracic spine. Clear posterior topography. This angle matters because posterior lung fields are where dependent ventilation and many clinically relevant processes declare themselves, from aspiration-related consolidations in the lower lobes to posterior pleural effusions layering along the costodiaphragmatic recess. For procedural planning, it also supports teaching surface anatomy for posterior auscultation and for pleural access, where intercostal spaces are approached posteriorly with careful attention to rib orientation and the neurovascular bundle on the inferior rib margin. The lower lobe emphasis is the point.
