Posterior anatomy frames the major internal organs against the axial skeleton and paraspinal soft tissues. The vertebral column runs in the midline with ribs curving laterally, while the lungs occupy the superior thorax on either side of the spine and the heart sits anterior to the thoracic vertebrae, only partially suggested from behind. Inferior to the diaphragm, the liver mass lies predominantly on the right, the spleen on the left, and the stomach and pancreas are positioned more anteriorly, with both kidneys and suprarenal (adrenal) glands appearing retroperitoneal and close to the posterior abdominal wall. Pelvic viscera of the adult female, including the uterus and ovaries, lie deep within the bony pelvis, inferior to the abdominal cavity and anterior to the sacrum and coccyx. Emphasis on posterior relationships makes this angle useful for correlating surface landmarks with deep structures during posterior and posterolateral approaches. For example, the kidneys project from approximately T12 to L3 with the right typically slightly inferior, a relationship that matters in percutaneous nephrostomy, posterior flank incisions, and interpreting posterior abdominal pain patterns that may reflect renal colic. Proximity of the spinal cord, exiting spinal nerves, and paravertebral vessels to the thoracoabdominal viscera also supports teaching of referred pain and autonomic pathways (splanchnic nerves and sympathetic chain) that clinicians map during regional anesthesia planning.
