Description

Along the inferolateral neck, the greater supraclavicular fossa forms a distinct depression immediately superior to the clavicle and lateral to the sternocleidomastoid muscle. Its floor is shaped by the inferior belly of the omohyoid and the prevertebral fascia overlying the scalene muscles, while the trapezius border contributes laterally as the neck transitions onto the shoulder girdle. Deep within this interval, the subclavian vessels course posterior to the clavicle as the brachial plexus trunks emerge between the anterior and middle scalene muscles. Surface contour matters. Clinically, this region is a key landmark for supraclavicular brachial plexus block, where needle trajectory is planned relative to the clavicle and the lateral border of sternocleidomastoid to reach the plexus at the level of the trunks while avoiding the subclavian artery and pleura. The greater supraclavicular fossa is also where Virchow (left supraclavicular) lymph nodes become palpable in metastatic disease, and where soft tissue fullness from hematoma, lymphadenopathy, or apical lung pathology can blunt the normal hollow. For line placement and trauma assessment, appreciating what lies deep to the fossa prevents false confidence from a superficial landmark.
